Health Promotion Plan

Three-part paper with the following instructions and separate parts that need to be separate Word documents.

Formally Written Paper

Step 1.) Select a client and health care promotion concern.
The client may be a family member or friend. Identify this client by age and initials only in the paper. Identify language, cultural background, and developmental age, but withhold full identification of the client. Then, identify the health promotion concern of the client.
Step 2.) Select an assessment technique or tool.
Select an assessment tool (for example.. Pender’s HPLP II-the Health Promotion Lifestyle Profile Survey to evaluate health promoting behaviors, PSS-Perceived Stress Scale to evaluate perceived stress) to gather data about the identified health promotion concern.
Describe the assessment tool: (example the HPLP II (if you used the HPLP II to evaluate health promotion behaviors–this is where you describe it) and cite your source.
Step 3.) Collect the data, evaluate and describe findings.
Collect the data from the assessment tool and describe the findings. Identify the abnormal findings or concerns of the client. (Ex: what are the results of your clients score on the HPLP II? What categories were out of range, high or low? This helps you identify needs and plan for the interventions that you will choose to improve health promotion behavior/s and essentially help your client work towards improving health long term.
Step 4.) Prioritize the data and recommend the desired outcome.
Collaborate with the client and interpret the data. Describe the key points in the process of collaboration. Prioritize the data and collaborate with the client to identify desired outcomes.
Step 5.) Recommend interventions.
Recommend evidence based interventions to meet the desired outcome. For example, if the client wants to start a walking plan three times a week, helping the client determine what the best approach would be a type of intervention (based upon best practice, cite sources to validate your rationale).     Step 1.) Review your part 1 paper.
    Reflect on the age of the client, their culture, literacy level, the health promotion problem and interventions identified.
    Step 2.) In this writing assignment, you will discuss the social support systems, health literacy, and access to resources necessary to implement the intervention/s for the client/s identified in the Module 1 paper.
    Write an Implementation Proposal and include all criteria below:

A. Introduction

  • State the purpose of the proposal.
  • Explain the background or context (why this implementation is needed).
  • Identify the target population or setting.

B. Problem Statement

  • Clearly define the issue the proposal addresses.
  • Use data or evidence to support the need for change.

C. Goals

  • List specific, measurable goals for the implementation (Use S-M-A-R-T Goals).

D. Proposed Intervention

  • Describe the strategy or program to be implemented.
  • Include:
    o Activities and steps
    o Resources needed (staff, materials, technology)
    o Timeline for implementation
    o Discuss the social support systems, community, health literacy, and access to resources necessary to implement the interventions identified in the Module 1 paper.
    o Provide thorough rationales.

E. Theoretical Framework

  • Explain which nursing or behavioral theories guide the proposal.
  • Justify why these frameworks are appropriate.

F. Implementation Plan

  • Detail how the intervention will be carried out:
    o Roles and responsibilities.
    o Communication plan/health literacy needs.
    o Training requirements for the patient if necessary.
    o Time line.
    Part 3: Evaluation & Sustainability Proposal and FINAL HEALTH PROMOTION PLAN

    B. Merge Part 1, Part 2, and Part 3 into ONE (1) Final Document to create a COMPLETE Health Promotion Plan. Submit your Health Promotion Plan for grading. Include a Title Page and Reference list. (Be sure each item in the reference list has a corresponding in-text citation within the paper).
