Case Study: Clinical Reasoning Client Report Clinical scenario/patient context: Mrs Huang is a 79 year old woman who lives at home with her daughter and grandchildren. She has been admitted to your facility for respite care whilst her daughter who is her primary carer has a 6 week holiday. She weighs 45 kilograms and has paper thin skin. She is in the early stages of dementia and you notice she does not eat or drink very much. Her daughter tells you she often gets very red and sore genitalia from urinary incontinence. She also tells you Mrs Huang is very unsteady on her feet and tends to pull back on her walking frame. Read the scenario above and write a 1200 word clinical reasoning patient report that addresses each of the following requirements as per the directions provided.
